Occupational Health : HGV/LGV Medicals

Who needs a Medical Report for an HGV/LGV/PSV Licence?
The Medical Examination Report form D4 has to be completed by:

  • Anyone who is making their first application for a license to drive a vehicle over 3.5 tonnes or a public service vehicle with more than 8 seats;
  • Anyone renewing their LGV/PSV entitlement aged 45 or over;
  • Anyone aged 45 or over will have to renew their license every 5 years, then every year after the age of 65.

Some medical conditions will result in a licence being granted for 1, 2 or 3 years. The DVLA should give notice prior to the renewal date.

Medicals usually take 60 minutes and involve a long questionnaire followed by an eye test, blood pressure measurement, urine test and a full physical examination of heart, lungs, circulation, nervous system, musculoskeletal system and abdomen.

The D4 form will be completed after the medical examination ready to return to the DVLA. The individual will also be told if there have been any abnormal findings which might affect the issue or renewal of the license.

Preparation for the medical
One of the most frequent causes of failing a medical is poor eyesight. If you think your eyesight is not as good as it might be, it could save you from failing the Medical by having your eyes tested by an optician before the Medical is due. If you are 45 years or older, or your eyes haven't been tested for two years or more, it would be wise for an optician to test your eyes anyway.

Medical conditions highlighted in the medical
On occasions the examining doctor may find something relating to the health of an individual which causes concern. If this is the case a letter will be given to the individual to take to their own GP, drawing attention to any specific signs observed during the Medical.
